There are three Hales listed on the Three Creeks Monument: J.E. Hale of the 73rd Ind. Inf., J.A. Hale of the 99th Ind. Inf., and J M. Hale of the 151st Ind. An 1872 Lowell Star news article lists J.E. as James E. Hale of the 73rd and J.A. as John A. Hale of the 99th. That leaves only J.M. Hale who might or might not be the same as the J.H. Hale of the following undated obituary from The Times, which was found in the Local History Files at Lowell Public Library (LH--Vital Statistics, v.3, p. 28):
TWENTY YEARS AGO
LOWELL -- Jerry H. Hale, veteran of the Civil War, and a resident of Lowell since it was a town, was found dead in his bed at his home here yesterday morning by one of his daughters. The aged man who has been in feeble health for several years, was feeling better than usual the last few weeks and his sudden death comes as a great shock to his family. His wife died less than a month ago.
